Supported Community Living

CCOI helps adults with intellectual and developmental disabilities live as independently as possible in the home of their choice. Service includes support with safety, housekeeping, finances, communication, employment, socialization, transportation and wellness.

Mental Health Services

CCOI serves the needs of Iowans with the functional limitations typically associated with chronic mental illness. Habilitation Services are designed to assist participants in acquiring, retaining and improving the self-help, socialization and adaptive skills necessary to reside successfully in home- and community-based settings. 

Residential Care

CCOI provides accommodation, board, personal assistance and other essential daily living activities for a period exceeding 24 consecutive hours. Veterans or individuals living in a residential care facility are unable to sufficiently or properly care for themselves because of illness, disease, or physical or mental infirmity

Day Habilitation

CCOI’s Day Habilitation Program helps individuals with disabilities and chronic mental health participate actively in a program designed with community participation, where they can learn, grow and form lasting friendships. Activities are tailored to each participant’s needs and interests and may include but are not limited to:

  • Community outings – Participants enjoy structured outings to locations such as parks, the movie theatre, and many out of town trips.

  • Music therapy – Participants learn to play instruments and participate in fun group activities such as talent shows.

  • Sporting – Participants improve motor, social and cognitive skills through outdoor sports.

  • Exercise – Group exercise time helps develop social skills and physical fitness.

  • Art – Participants learn to express themselves through crafts and other creative projects.

Host Home

Host Homes provide individuals with disabilities the opportunity to live in a safe, supportive, and family-centered environment tailored to their needs. For providers, it’s a meaningful way to make a difference—offering independence to others while earning competitive compensation as an independent contractor. Each match is carefully guided to ensure compatibility, respect, and shared values. Community Care of Iowa supports both clients and providers with assistance, training, and resources to ensure long-term success.
